I hereby give permission [for my student] to participate in all of CSCR’s scientific, civic, and educational activities in and around its campus, including on the water activities, shoreline activities, and those activities that engage students with community stakeholders, either in person, or electronically. I understand that CSCR digitally records many of its activities and that photos, videos, and audio recordings that include me or my student may be shared with stakeholders or used for CSCR’s advertising and marketing purposes. In these contexts, I consent to the use of digital representations of me and or my student in which I or my student may be digitally present, visible, and identifiable. I understand that the pursuit of scientific and civic study may include transportation over land by foot or by automobile, as indicated to access CSCR study sites described generally or specifically in program and project materials. I understand transportation to and from CSCR and study sites of educational and scientific value may include transportation by power boat, or by canoe or kayak. In the event of a student emergency at which I am not present, I understand every effort will be made to contact me or my designee, using the phone numbers and emails provided during registration. I further understand that efforts to contact me will be made while prioritizing communications to engage 911 services. In the event I or my spouse, partner, or designee cannot be reached, I hereby give permission to CSCR to engage professionally trained and certified emergency medical service providers to secure proper medical treatment, including as is deemed necessary by said providers, hospitalization, anesthesia, surgery or injection of medication for my student. For my student, our personal representatives, family, heirs and assigns, and for myself, I knowingly and freely waive all claims against and release and discharge CSCR and its officers, directors, agents, independent contractors, employees and volunteers from any and all liability, loss, damage and expense which may result from participation in CSCR programs
